The applicant is required to fill up Form ‘A’ in the prescribed format for obtaining new licence under Shop Act & Establishment Act 1948, and after verification of this form a receipt is issued by Citizen Facility Centre.  Thereafter, on inspection of the site as per the details shown in the form by the Inspector, the procedure for issuing new licence is taken up.

If the demand is for new licence, Copy of the construction and completion certificate from the concerned department of Municipality and copy of the Municipal taxes having paid and as documentary evidence Deed or Allotment letter and copy of the Index or survey are to be produced.  And, if the licence has been issued for the said place previously, the same is to be cancelled.

As per Rule-10, when there is change in ownership or partnership or change in occupation or place in Licence, on submission of Form ‘E’ duly filled in alongwith necessary documentary evidence, on verification and site inspection if required, the change is made in Form ‘E’ as per demand.
